Breathing freely again at last
A patient came to us with breathing difficulties after an unsuccessful first operation — we reconstructed the framework and the function with cartilage grafts.
The starting point
Unfortunately nose operations do not always work out. There is no surgeon in the world who can honestly say: “All my noses turn out well.” The reasons for that are anatomical realities and forces during the healing process that cannot always be calculated. Frequently, however, it is a matter of technical shortcomings during the operation — the result then does not match the aesthetic expectations, and the function of the nose is impaired. These problems have to be corrected again.
Irmgard S. (35) was very disappointed after her first nose operation. Not only was the nose unattractive — she had also suffered a collapse of the internal and external nasal valves. As a result her nasal breathing was considerably impaired; she could no longer breathe strongly and freely. Her treating doctor had reached the limits of his competence and referred her to Dr Kuzbari. Irmgard S. was greatly relieved when he told her that the condition could be corrected.
The operation
First the framework of the nose had to be reconstructed. That required cartilage to be grafted, because many parts of the cartilage framework had been surgically removed or deformed by scarring. Since the septum had not been touched during the first operation, there was expendable cartilage material there that could be used.
Cartilage grafts were placed at invisible points to support the columella and to widen the bridge, which had become too narrow — which made the nose look more natural again. At the same time this opened the internal nasal valve, so that breathing became freer. With several visibly placed cartilage grafts the nasal tip, deformed by the previous operation, was reconstructed. These grafts also stabilised the nose, to prevent it from being pulled out of shape during the healing process.
The result
Just a few weeks after the operation a harmonisation of the shape of the nose was clearly visible. Once the swelling had fully subsided, the corrected nose looked unobtrusive and harmonious. The nose was no longer a conspicuous eye-catcher but fitted harmoniously into the face. Unrestricted breathing was possible again too.
“With my corrected nose I have regained my joy in life.”
Technical shortcomings after previous surgery can be corrected — but the effort involved is considerably greater than with a first operation. And that is exactly where the focus of our work lies: bringing noses that have already been operated on elsewhere back into form and function, with a calm hand, experience and honest advice.
