Bone, cartilage, soft tissue.
As an oral and maxillofacial surgeon, DDr Wittmann does not think of the nose in isolation but as part of a framework: nasal bone, septum, alar cartilages and the soft tissue above them together determine shape and function.
Anyone who knows these layers can predict how a change to the cartilage will show on the surface months later. That is precisely the difference between a nose that still fits after a year and one that warps.
DDr Wittmann completed her specialist training in the minimum time and in doing so gained a deep understanding of how the bony, cartilaginous and soft structures of the face interact and affect shape, appearance and function. Her additional degree in dentistry added further facets to this knowledge and sharpened her feeling for aesthetics in the facial region.
Nose-bridge-preserving technique and a cartilage-supported nasal tip.
Where the starting findings are suitable, DDr Wittmann works with the modern nose-bridge-preserving technique known as dorsal preservation, as well as with hybrid techniques — not exclusively with traditional structural septorhinoplasty. The natural nasal bridge is preserved instead of being taken down and rebuilt.
This has three advantages: the result looks particularly natural, it is stable in the long term, and the risk of later irregularities on the nasal bridge is reduced — which counts above all with thin skin, because there even the smallest edge becomes visible. Certain deviations also recur less often. Whether the technique is an option for your nose is shown by the examination in the initial consultation.
We stabilise the nasal tip using modern cartilage-supported techniques. This significantly reduces the risk of drooping of the nasal tip over the years. Cartilage from your own body is used for this, as a rule from the septum.
“Particularly in nose surgery, but also to some extent in eyelid surgery, function — nasal breathing and field of vision — can often be improved considerably in the same breath, which makes aesthetic surgery even more rewarding,” says DDr Wittmann about her work. Projectometer in theatre
During her nasal operations DDr. Wittmann uses the projectometer to measure the projection of the nasal tip and the height of the bridge — before the first step and after every further one, each time against the plan from your 3D simulation. What happens in theatre can therefore be checked, not merely estimated.
Measuring instead of estimating.
The projectometer is a fine stainless-steel measuring instrument placed directly on the nose within the sterile field. It carries a millimetre scale, an adjustable arm and a protractor, and with them it records the two values that determine the profile of a nose: how far the nasal tip projects forward — the projection — and how high the bridge sits.
DDr. Wittmann takes the baseline measurement before the first step. After every further step — after the bridge has been lowered, after a support has been placed at the tip — she measures again and compares the figure with the plan drawn up during the initial consultation on the 3D simulation. Whether the bridge has been reduced by the planned amount and the tip raised accordingly is therefore settled in theatre as a measured value.
The classification matters to us: the projectometer makes no decisions. Planning and experience remain the surgeon’s achievement. The instrument checks them — and makes it possible to follow whether what happens in theatre matches what you saw on screen beforehand.
Techniques from Brazil.
In 2026 DDr. Wittmann completed a fellowship in Brazil together with Dr. Christoph Grill, working intensively with new and modern techniques of rhinoplasty — with a focus on thick skin and on ethnic nasal shapes. These techniques now feed into her work at the Kuzbari Centre.
With thick skin, a broad bony base or a tip with little definition, the approach is to build up rather than take down: a stable cartilage framework carries the tip against the skin and gives it from within the definition that cannot be worked out from the outside. The character of your nose is preserved in the process.
